Sec. 11.
"Case management" means the management of patient activities identified in the individualized treatment plan that assist in patient goal attainment, including referrals to other service providers, such as mental health, physical health, housing, education, vocational rehabilitation and other employment services providers, and linking patients to recovery support groups.
