Section 410 IAC 3.2-1-2 - "Care coordination" defined

Sec. 2.

"Care coordination" means providing assistance necessary to help ensure the effective and efficient organization of and access to services and resources that are appropriate and necessary to meet the needs of a child with special health care needs and the child's family.
