Section 410 IAC 1-2.2-8 - Registry

Sec. 8.

The department shall establish a confidential registry of all persons submitting written requests pursuant to section 6 of this rule. The registry shall include the following information about the physician:

(1) Full name.
(2) Street address.
(3) City.
(4) Zip code.
(5) County.
(6) Telephone number.
