Section 405 IAC 5-21.8-1 - General provisions

Sec. 1.

The intent of this rule is to provide BPHC services to individuals with serious mental illness who demonstrate impairment in self-management of health services. Eligibility for services and the provision of services are based upon an individual meeting specific BPHC needs-based and targeting criteria. BPHC services will be:

(1) provided through a state plan; and

(2) delivered by service provider agencies meeting specific state-defined criteria.
