Sec. 17.
(a) A wastewater treatment plant operator certificate: (1) is issued after an applicant successfully completes the examination for the appropriate classification;(2) must specify the: (A) month and year that the applicant qualified; and(B) date the certificate was issued;(3) must be validated by a current certification card; and(4) is not valid if obtained through:(C) the submission of incomplete or inaccurate data on the examination application.(b) A certification card: (1) may be issued for a time period of not more than thirty-six (36) months; and(2) expires on the date printed on the card.(c) To obtain a replacement or duplicate certificate, a certified operator shall submit a written request to the commissioner, including the following information: (1) The class of the wastewater treatment plant operator.(2) The date of issuance of the original certificate, if known.(3) The certificate number.Water Pollution Control Division; 327 IAC 5-23-17; filed 11/6/2023, 10:44 a.m.: 20231206-IR-327180365FRAReadopted filed 10/18/2024, 1:57 p.m.: 20241113-IR-327230810RFA
