Sec. 7.
(a) A training course provider seeking reapproval of an approved initial training course or an approved refresher training course by the department shall meet the following requirements:(1) Have possessed a valid letter of approval from the department within the previous six (6) months.(2) Submit a completed application on forms provided by the department and include updated information as required in section 6(a)(2) and 6(a)(3) of this rule.(3) Pay the annual application fees as specified in section 11(b) of this rule.(b) The department shall review the application and make a determination as to the eligibility of the training course provider.
