Sec. 15.
(a) All attorneys who appear in a representative capacity on behalf of a party must file written notice of appearance setting forth the following: (1) The:(E) electronic mail address; of the attorney. (2) The name and address of the party.(3) The Indiana attorney number.(4) If not licensed in Indiana, the following:(A) A verified statement that the attorney is in good standing.(B) A designation of the jurisdiction in which the attorney is currently licensed to practice law.(C) The attorney registration number.(5) If an attorney files a petition for review of behalf of his or her client that contains the information required by subdivisions (1) through (4), the petition shall serve as a written notice of appearance.(b) A representative that is not an attorney of a party must file written notice of the representation. Nothing in this subsection relieves a person from compliance with Rule 5.5 of the Indiana Rules of Professional Conduct.
