Section 312 IAC 29-25-3 - Reporting of fires and blowouts

Sec. 3.

(a) In addition to the reporting requirements for spills in section 2 of this rule, an owner or operator shall report all fires or any blowout or other incident involving the loss of control of a well or any facility associated with the drilling operation or plugging of wells for oil and gas purposes to the division within two (2) hours of discovery by telephone to the inspector, and by email to spill@dnr.in.gov.
(b) The report shall include the following information:
(1) The location of the fire, or blowout by lease name, if known, and directions to the site.
(2) The name of the owner or operator of the well or facility involved.
(3) Whether local emergency response personnel have been notified.
(4) Emergency measures taken or in progress to contain or control the incident.
(c) Within thirty (30) days following the incident, the owner or operator shall file a final report on a form prescribed by the division identifying:
(1) details of the incident;
(2) factors contributing to the incident; and
(3) preventive measures that have or will be taken to protect against similar future incidents.
