Required consumer records for licensed hearing instrument dispensers/audiologists shall be copies of medical evaluations, medical waivers, all contracts, receipts, and audiometric test results (audiograms).
a) The full name of the licensed hearing instrument dispenser/audiologist and the date of the test shall be recorded on the audiogram.b) When a hearing instrument is sold, as defined in Section 3 of the Act, copies of all records that are required in this Section shall be retained at the place of business shown on the contract for a minimum of 36 months.
