Section 24.08.01.454 - RECORDS OF BODIES01.Content of Record. Each funeral establishment and crematory must maintain a record of each burial, cremation, or other disposition of human remains, disclosing: a. The name of the decedent; andb. The name and address of the person, or names and addresses of the persons if more than one (1), authorizing the burial, cremation, or other disposition of that body; andc. A statement as to whether or not the body was embalmed; andd. The date of the burial, cremation, or other disposition of that body; ande. The subsequent disposal of any cremated remains.Idaho Admin.
