Section 02.04.05.221 - LABORATORY LICENSING REQUIREMENTS01.License Required. All laboratories that test bovine milk or cream components and quality parameters for a basis of payment must be licensed by the department as an official laboratory.02.License Application. A laboratory must apply for a license on a form prescribed by the department. The laboratory must identify (on the application form) the names of all persons who will test milk or cream components and quality parameters.03.License Fee. The license fee, per laboratory, is twenty-five dollars ($25).04.License Term. The official laboratory license is valid for three (3) calendar years after issuance by the department, unless otherwise suspended or revoked in accordance with these rules.
