Section 16-99-106 - Forfeited registration, certification; restoration
(a) Failure, neglect, or refusal by any person to pay the respective biennial renewal provider registration fee or course certification fee shall constitute a forfeiture of the certificate of registration or certification.
(b) Any forfeited certificate of registration or certification may be restored upon satisfaction of all of the following:
(1) Written application;
(2) Continued satisfaction of the respective requirements of sections 16-99-99 and 16-99-100; and
(3) Payment of the applicable fees specified in rules adopted by the director of commerce and consumer affairs pursuant to chapter 91, HRS.
