Section 15-219-47 - Contents of contested case petition(a) The petition shall state the following: (1) Name, address, and telephone number of the petitioner and the petitioner's legal counsel, if any, which shall be updated by the petitioner at all times;(2) A brief and concise factual statement of the petitioner's claim;(3) The law or rule which affords petitioner the right to a contested case hearing;(4) The names of all respondents or identities against whom the petition is brought; and(5) A brief statement of the relief sought by the petitioner.(b) If the petition is not in substantial compliance with subsection (a), the authority may request that the petitioner submit an amended petition in compliance thereto.
