Section 12-14-30 - Closing report(a) The provider shall submit a closing report on each case upon the termination of vocational rehabilitation services. The report shall include, but not be limited to, the following: (1) Justification for closure;(2) A brief description of services provided;(4) Employment information: (B) Date employment began; and(C) Wage information; and(5) Signature of the employee "witnessed by" the specialist, including a statement the report has been reviewed with the employee.(b) Specialists shall not determine the employee is able to work unless they can document their ability to secure for the employee suitable gainful employment.(c) Should the employee appear medically able to return to employment but not be employed at the time of vocational rehabilitation case closure, the employee may be referred to another provider for vocational rehabilitation services.(d) If no objection is received by the director within ten calendar days from the date of the director's closure notice, the closure will be considered final and the employee may not request further reconsideration under section 12-14-48. The front page of the objection shall be clearly identified as a "VOCATIONAL REHABILITATION CLOSURE OBJECTION" in capital letters in no less than ten point type.[Eff. 1/28/85; am 4/12/93; am 11/08/99; am 12/17/01] (Auth: HRS § 386-72) (Imp: HRS § 386-25)
