Each document filed with the agency by any person shall include the person's name and mailing address. The document shall be signed in ink by the person or the person's authorized agent or attorney. The signature of the person signing the document constitutes a certification that the person has read the document; that to the best of the person's knowledge, information, and belief, every statement contained in the document is true and no statement is misleading; and that the document is not interposed for delay. Each document shall be plainly legible.
