Section 5C-24.002 - General Requirements(1) Forms Provided. The Division of Animal Industry, Department of Agriculture and Consumer Services will provide forms when required by statute or rule for certification of identification, required tests, and health as required for movement, exhibition, and other designated purposes for the species of animal.(2) Fees for Forms. A fee will be charged for the forms as provided in Rule 5C-24.003, F.A.C. The fee must be submitted with the request for forms, as a check or money order made payable to the Florida Department of Agriculture and Consumer Services.(3) Deposit of Fees. The fees collected shall be deposited in the Department's General Inspection Trust Fund.Fla. Admin. Code Ann. R. 5C-24.002
