Each application for a hacker's license shall be made on a form provided by the Chairperson.
Each application shall set forth the applicant's full lawful name (including middle name, or names, if any and any other names by which the applicant has been known), date of birth, sex, social security number, residence, and other information that the Chairperson may require to determine the applicant's identity, competency, bona fide residency and eligibility, including a record of all criminal and traffic charges entered against the applicant in the District and elsewhere and local and federal income tax filings.
Each application should be accompanied by two (2) new full face and one (1) profile head and shoulders photographs, size one and three-quarter inches by one and seven-eighths inches (1 in. x 1 in.).
Each application shall also be accompanied by three (3) sheets of fingerprints of the applicant taken at the headquarters of the Metropolitan Police Department. One (1) sheet of fingerprints shall be furnished to the Federal Bureau of Investigation (FBI).
Each application shall also be accompanied by individual letters from the applicant's most recent employer and from at least three (3) responsible residents of the Metropolitan Area who are engaged in a business or profession, who have known the applicant for a period of at least one (1) year, and who shall vouch for the sobriety, honesty, and general good character of the applicant. Each letter shall include the signature and the current address of the writer.

The Chairman may retain a portion of the license fee, not to exceed twenty-five percent (25%), for the administrative costs of processing applications that are denied after intake and processing.
A false statement made in the application may result in denial of application for licensure or subsequent suspension or revocation of the license once issued.
