Services My Way participants may decide at any time to voluntarily terminate their participation in the Services My Way program.
A Services My Way participant shall indicate the decision to voluntarily terminate participation in the program by completing and submitting any required voluntary termination forms to DHCF or its designee. The participant's authorized representative and/or support broker shall assist the participant to complete the forms as necessary.
Upon receipt of the participant's voluntary termination forms, DHCF or its designee shall inform the participant's EPD Waiver case manager and support broker of the participant's decision to terminate program participation.
EPD Waiver case managers shall be responsible for assisting participants to transition to agency-based personal care aide services. EPD Waiver case managers shall ensure that there is no break in service provision during the transition period and shall coordinate the approval by DHCF or its designee of the request to initiate agency-based personal care aide services.
A participant's decision to voluntarily terminate participation in the Services My Way program does not affect their eligibility for EPD waiver services.
