Rule 25-F901 - REPORT OF FINDINGS - DOCUMENTING INFORMATION AND OBSERVATIONS901.1The Department shall document on an inspection report form:
(a) Administrative information about the tanning facility's legal identity, street and mailing addresses, inspection date, and other information such as status of the license and personnel certificates that may be required or other inspectional findings; and(b) Specific factual observations of violations of these regulations that require correction by the licensee including:(1) Nonconformance with critical items of these regulations;(2) Failure of a licensee to correct cited violations, as specified in Section 908 or 910; or(3) Failure of the licensee to ensure that operators are properly trained and have knowledge of their responsibility as specified in Chapter 2.D.C.
