Applications for specific licenses shall be filed with the Director on a form prescribed by the Director, and containing information that the Director may require.
At any time after the filing of the original application, and before the expiration of the license, the Director may require further statements in order to determine whether the application should be granted or denied or whether a license should be modified or revoked.
Each application shall be signed by the applicant or licensee or a person duly authorized to act for and on his or her behalf.
In the application, the applicant may incorporate by reference information contained in previous applications, statements, or reports filed with the Director; Provided, that the references shall be clear and specific.
An application may include a request for a license authorizing one (1) or more purposes.
Applications and documents submitted to the Director may be made available for public inspection, except that the Director may withhold all or part of any document from public inspection if disclosure of its contents is not required in the public interest and would adversely affect the interest of a person concerned.
