Rule 22-B1904 - RENEWAL OF PHARMACY LICENSE
1904.1

The Director shall mail a renewal notice to a licensee by first class mail to the licensee's last known address on file with the Director at least sixty (60) days prior to the expiration of the license.

1904.2

The failure of a licensee to receive the renewal notice required by this section does not relieve the licensee of the responsibility of renewing the license by the expiration of the existing license.

1904.3

A licensee applying for renewal of a license shall submit the application for renewal not less than thirty (30) days prior to its expiration, to avoid lapse.

1904.4

A licensee applying for a renewal of a license shall disclose whether the licensee is currently engaged in the compounding of sterile pharmaceuticals or intends to engage in the compounding of sterile pharmaceuticals after approval of a licensee.

1904.5

If the Director does not receive the application for renewal of a license at least thirty (30) days prior to the expiration date, the license shall lapse on the expiration date. The licensee may be reinstated within thirty (30) days of expiration upon receipt of a completed renewal application and the payment of a late fee.

1904.6

Upon receipt of the required late fee and final processing of the renewal application, the licensee shall be deemed to have possessed a valid license during the period between the expiration of the license and the reinstatement date.

1904.7

Reinstatement of a license that has been expired for over thirty (30) days shall be at the discretion of the Director. Otherwise, a licensee that fails to submit the completed renewal application or required late fee within thirty (30) days after the expiration of the applicant's license shall be required to apply for new licensure pursuant to § 1902 of this chapter.

1904.8

Prior to the renewal of a license, the Director shall make an inspection of a pharmacy to determine compliance with the Act and this chapter.

1904.9

The Director shall send a written report of the findings of the inspection to the licensee no later that fifteen (15) days after the conclusion of the inspection.
