3908.1 Referrals to the Clubhouse may be made by a CSA, family member, advocates, other service providers, or by the consumer.
3908.2 Referrals from a CSA shall be made in writing and include the following information:
(a) Current Plan of Care;(b) Current behavioral health assessment;(c) Contact information for the consumer, including emergency contact information (family member, friend or guardian as applicable);(d) Crisis Plan for the consumer (if available); and(e) Advance Directives or instructions as described in § 3405.6 of this subtitle (if available).3908.3 Self-referrals, referrals from a relative, other service provider or advocate shall be made in writing and include the following information:
(a) Name of the person's CSA or current behavioral health service provider (if applicable);(b) Current behavioral health assessment (if available);(c) Contact information, including emergency contact information (relative, friend or guardian as applicable);(d) Crisis Plan (if available); and(e) Advance Directives or instructions (if available).3908.4 A person enrolled with a CSA must have a Diagnostic Assessment and a Plan of Care that includes Clubhouse services in order to participate in the Clubhouse.
