Section 17b-262-835 - Plan of care(a) The interdisciplinary team in conjunction with the attending physician shall establish an initial written plan of care for each client within 48 hours of the client's election of hospice. Services may not be billed until the plan is established.(b) The interdisciplinary team, in collaboration with the individual's attending physician, if any, must review, revise and document the individualized plan as frequently as the client's condition requires, but no less frequently than every 14 calendar days.(c) The plan of care shall specify the care and services necessary to meet the client's and family's needs identified in the comprehensive assessment.Conn.
