Section 17b-262-803 - Other(a) Where brand names or stock or model numbers are specified on the prescription or the PA, no substitution shall be permitted without the written approval of the department.(b) The provider shall instruct the client, his or her family or a designated representative on the proper use and care of the hearing aid.(c) The provider shall maintain a written usual and customary price list that details individual product and service charges. This list, including updates along with any required manufacturer's list pricing, shall be available for review by the department.(d) A hearing aid purchased by the department shall become the property of the client on the date of delivery to the client.Conn.
