Section 17b-262-826 - Billing requirements(a) Claims shall be submitted by the providers on the department's designated form or electronically transmitted to the department's fiscal agent and shall include all information required by the department to process the claim for payment.(b) The provider shall bill its usual and customary charge for the services delivered, except as set forth in section 17b-262-827(b) of the Regulations of Connecticut State Agencies.Conn.
