Section 4 CCR 734-1.8 - REPORTING CHANGE OF ADDRESS, TELEPHONE NUMBER, OR NAME (C.R.S. sections 12-245-204, 12-245-206)A. Unlicensed psychotherapists shall inform the Division of any name, telephone number or address change within thirty days of such change. Staff shall not change an unlicensed psychotherapist's information without written notification from the unlicensed psychotherapist. Notification via mail, fax, e-mail, and the online system is acceptable. Verbal notification is not acceptable.B. Any of the following documentation is required to change an unlicensed psychotherapist's name or correct a social security number or individual taxpayer identification number: marriage license, divorce decree, court order, or documentation from the Internal Revenue Service verifying the licensee's valid individual taxpayer identification number.
