Section 3.08 - Applicant and Licensee Reporting of Status Changes

It is the responsibility of the applicant or licensee to notify the Board in writing of any of the following events within 30 days of their occurrence, unless sooner required under CMR 4.03(3):

(1) Any change in the address of record;

(2) Any change of name; or

(3) The permanent departure from the Commonwealth of Massachusetts.
