The rental of any of the following requires prior authorization:
(A) oxygen or respiratory therapy delivery systems;(B) suction apparatus;(C) nebulizers;(D) intermittent positive pressure breathing (IPPB) machines; and(E) equipment that is not listed as a rental item in Subchapter 6 of the Oxygen and Respiratory Therapy Equipment Manual.
