Section 180.285 - Standard - Specimen RecordsThe laboratory shall maintain a record indicating the daily accession of specimens, each of which shall be numbered or otherwise appropriately identified. Records shall contain the following information:
(A) The laboratory number or other identification of the specimen.(B) The name and other identification of the person from which the specimen was taken.(C) The name of the licensed physician, other authorized person or clinical laboratory which submitted the specimen.(D) The date the specimen was collected by the physician or other authorized person.(E) The date the specimen was received in the laboratory.(F) The condition of unsatisfactory specimens when received (e.g. broken, leaked, hemolyzed, turbid, etc).(G) The type of test performed.(H) The date the test was performed and the name or initials of the person performing the test.(I) The results of the laboratory test or cross-reference to results and the date of reporting.(J) The name and identification of the laboratory the specimen is referred to if the procedure is not performed on the premises.
