(a) Each program shall submit in its protocol a plan for evaluating the effectiveness of the program which: (1) States realistic and clearly defined objectives.(2) Shall outline in detail the methodology to be employed.(3) Specifies all data to be collected.(b) Each program shall insure that the necessary information is collected and recorded in a uniform manner before initiation of treatment and at predetermined intervals during and after termination of treatment.(c) Each program shall verify required information supplied by the patient, when it is possible to do so.Cal.
