Any person affected by a violation of any term or condition of a permit or license may file a complaint with the board. The complaint shall contain the following:
(a) Name and address of the complainant.(b) The nature and location of the alleged violation.(c) The manner in which the complainant was affected.(d) A statement of whether complainant is required by Part 5.1 (commencing with Section 5100) of Division 2 of the Water Code to file with the board a statement of his water diversion and use, and if complainant is so required, a statement: (1) That complainant has filed with the board a statement of water diversion and use; or(2) Setting forth the reason for complainant's failure to file such a statement of water diversion and use.(e) The specific action which complainant requests. Any person may complain of a violation of a condition intended for the benefit of the public.
