Section 70736 - Sterilization Reporting Requirements(a) All hospitals performing tubal ligations, vasectomies, and hysterectomies shall submit to the Department a quarterly report containing the following information: (1) The total number of such sterilizations performed, including diagnoses and types of procedures employed.(2) The number and type of such sterilizations performed by each physician on the medical staff preserving the anonymity of the physicians and patient.(3) Demographic and medical data as required by the Department.Cal.
