The appeal shall be typewritten and may be in letter or other written format. The appeal shall state that it is an appeal under Article 2 of Chapter 2 of Division 5 and shall include all of the following:
(a) The permit for which the Department or local agency applied and whether the permit was issued or denied;(b) The statute, ordinance, regulation or rule pursuant to which the permit may be issued;(c) If the permit was issued, the conditions determined by the Department or local agency to be unreasonable and the reasons why such conditions would lead to a significant delay in a seismic retrofit project;(d) A concise statement of the necessity of the project for the preservation of the public peace, health, or safety within the meaning of Article IV of the California Constitution; and(e) If the permit was applied for by a local agency, the name, address, telephone number, and fax number, if any, of the local agency.Cal.
