Section 125.21 - Installation and Removal Request(a) An installer shall verify eligibility with the department prior to installing or removing an ignition interlock device by one or more of the following methods: (1) Contact the Mandatory Actions Unit using the telephone number provided by the department.(2) Submit an Ignition Interlock Device (IID) Installation & Removal Request, form DL 925 (Rev. 1/2019), which is hereby incorporated by reference, and fax to the number provided on the form.Cal.
