Appendix B

Disclosure Categories

Designated employees in Category 1 shall report the following:

* Investments, business positions in business entities, and income, including receipt of gifts, loans, and travel payments, from sources, which, within the preceding 24 months, have been subject to regulation, licensing, enforcement, or oversight by or under the jurisdiction of the Department.

Designated positions in Category 2 shall report the following:

* Investments, business positions in business entities, and income, including receipt of gifts, loans, and travel payments, from sources, of the type that provide goods, services, or equipment that the Department utilizes or currently plans to utilize.

Designated positions in Category 3 shall report the following:

* Investments business positions in business entities, and income, including receipt of gifts, loans, and travel payments, from sources, that are required to be licensed by the Department, or that provides education or training, which, within the preceding 24 months, have been required by the Department to qualify for or maintain a license.

Designated positions in Category 4 shall report the following:

* Investments, business positions in business entities, and income, including receipt of gifts, loans, and travel payments, from sources, which have been the subject of investigation, or reviewed by, the designated position during the past 12 months.

Designated positions in Category 5 shall report the following:

* Investments, business positions in business entities, and income, including receipt of gifts, loans, and travel payments, from sources of the type that provide goods, services, or equipment that the designated position's bureau, branch, division, or area of assignment utilizes or currently plans to utilize.

Designated positions in Category 6 shall report the following:

* All investments, business positions, and income, including receipt of loans, gifts and travel payments, from, sources of the type that sell, rent, or lease information technology equipment, software, facilities, supplies or services, to the Department, including California Municipal Award Schedule contractors.

Designated positions in Category 7 shall report the following:

* Investments, business positions in business entities, and income (except for income received solely from the sales of insurance products or solely for adjusting insurance claims), including receipt of gifts, loans, and travel payments, from sources that, within the preceding 24 months, have been approved by the Department as education providers.

* Investments, business positions in business entities, and income (except for income received solely from the sales of insurance products or solely for adjusting insurance claims), including receipt of gifts, loans, and travel payments, from insurance agents and brokers, adjusters and bail agents who are subject to the permit or licensing authority of, or have an application for a license or permit pending before, the Department.

Designated positions in Category 8 shall report the following:

* Investments, business positions in business entities, and income, including receipt of gifts, loans, and travel payments, from workers' compensation insurers.

Designated positions in Category 9 shall report the following:

* Interests in real property in the State of California.

This conflict-of-interest code, which was originally effective on 12-15-2005, and is, therefore, the most recently legally adopted code, replaces the voided conflict-of-interest codes (Register 2013, No. 10).
