(a) All applications for a home protection company license shall be filed with the Corporate Affairs Bureau, Legal Division, Oakland Office.(b) Submit an original and one copy of the application, index of exhibits and all exhibits.(c) The applications and exhibits must comply with the following requirements: (1) The documents should be arranged as follows: Application

(2) In the event that an exhibit is not submitted with the application, please so indicate.(3) Supplements or amendments to exhibits should be submitted and marked in the same manner as original exhibits. Each supplement or amendment should consist of a whole document or complete pages of a document paginated to correspond to existing exhibits so that the new material can be readily inserted into the existing file.(4) Every application will be assigned a file number. Applicants will be notified on the file number and filing date, and all subsequent correspondence shall bear the file number.(5) The application, amendments and supplements shall be verified as required by the Code of Civil Procedures Sections 446 and 2015.5.Cal.
