Section 2241.5 - Filing Procedure

U. C. D. policies and forms shall be submitted to the commissioner for filing with and approval by the commissioner in conformity with the requirements of Article 1 of this subchapter applicable to other disability policies and forms, except that in the submission of final copies for approval, each form shall be submitted in triplicate. In addition, one copy should be submitted to the Department of Employment Development, Division of Disability and Hospital Benefits, Sacramento, California.

1. Amendment filed 5-6-77 as procedural and organizational; effective upon filing (Register 77, No. 19).
2. Change without regulatory effect amending section filed 7-14-2021 pursuant to section 100, title 1, California Code of Regulations (Register 2021, No. 29). Filing deadline specified in Government Code section 11349.3(a) extended 60 calendar days pursuant to Executive Order N-40-20.
