The assignment referred to in Section 2122 shall be in writing and shall contain, in substance, the following provision:
"In consideration of allowing me credit for the full amount of the gross return premium on Policy No. __________covering my 19 ___ automobile, I hereby assign to __________ all my right, title and interest in and to such return premium."
A signed copy of such assignment shall be retained by the person taking the assignment in the records and files of the person's insurance transactions.
