An application for permission to intervene and become a party to a hearing shall be filed with the Commissioner and served on the respondent and each other party (other than the Commissioner) not less than five business days before commencement of the hearing, and shall contain the following:
(a) Description of the applicant's interest in the matters at issue in the hearing.(b) Brief summary of the evidence which the applicant intends to introduce and a list of the witnesses whom the applicant intends to present at the hearing.(c) Statement of the applicant's purposes in intervening in the hearing.(d) Statement of reasons why the intervention of the applicant in the hearing would be in the public interest.Cal.
