Section R9-7-535 - NotificationsA. In addition to the reporting requirements specified in Article 4, each licensee shall provide a written report to the Department if any of the following incidents involving radiography equipment occur: 1. Unintentional disconnection of the source assembly from the control cable;2. Inability to retract the source assembly to the fully shielded position or secure it in this position; or3. Failure of any component (critical to safe operation of the device) to properly perform its intended function;B. A licensee shall include the following information in any report submitted under this Section, regarding radiography equipment, or Article 4, regarding an overexposure, if the report concerns the failure of safety components of radiography equipment: 1. A description of the equipment problem;2. Cause of the incident, if known;3. Name of manufacturer and model number of the equipment involved in the incident;4. Place, date, and time of the incident;5. Actions taken to establish normal operations;6. Corrective actions taken or planned to prevent reoccurrence; and7. Qualifications of personnel involved in the incident.C. Any licensee that conducts radiographic operations, or stores radioactive material at a location not listed on the license or for a period longer than 180 days during a calender year, shall notify the Department of these activities before the 180 days has elapsed.Ariz.
