A. An applicant shall submit to the Department the following fee for: 1. A nonrefundable initial application, $100;2. An initial license for a regular or business hearing aid dispenser, $200;3. A renewal application for temporary hearing aid dispenser license, $100.4. A regular or business hearing aid dispenser licensee for a renewal license, $200.B. If a renewal application is submitted within 30 calendar days after the license expiration date, a licensee shall submit with the renewal application a $25 late fee.C. The fee for a duplicate license is $25.D. An applicant, who is not a business organization, for initial licensure is not required to submit the applicable fee in subsection (A) if the applicant, as part of the applicable application in R9-16-303 or R9-16-306, submits an attestation that the applicant meets the criteria for waiver of licensing fees in A.R.S. § 41-1080.01.Ariz.
