Section R4-45-104 - Change of Name or Address
A. A licensee shall notify the Board in writing within 30 days after the licensee's name is legally changed. The licensee shall include with the notice a notarized or certified copy of the official document evidencing the name change. At the time of notification, the licensee shall request a duplicate license in the new name and pay the fee prescribed in R4-45-102(A)(7).
B. A licensee shall notify the Board in writing within 10 days after a change in the licensee's address of record.
