Section R18-5-105 - CertificationA. The Department shall issue an operator certificate to an applicant if the applicant: 1. Meets the experience and education requirements in R18-5-112 for the applicable class and grade,2. Passes a written examination for the applicable class and grade, and3. Has not had an operator's certificate revoked in Arizona or permanently revoked in another jurisdiction.B. To apply for operator certification, an applicant shall submit or arrange to have submitted to the Department the following information, as applicable, in a format acceptable to the Department: 1. The applicant's full name, Social Security number, and operator number;2. The applicant's current mailing address, home and work telephone numbers, fax number, and e-mail address;3. The applicant's place of employment, including the facility identification number;4. The class and grade of the facility where the applicant is employed;5. Proof of successful completion of the examination for the applicable class and grade; and6. Documentation of the applicant's experience and education required under R18-5-112.Ariz. Admin.
