Section 800-6-7-.04 - PEEHIP Enrollment Forms

Separate forms for enrolling in each specific PEEHIP coverage (Cancer, Hospital/Medical, Hospital Indemnity, Dental). These forms were used during the initial enrollment period when PEEHIP was created (Fall 1983) and subsequent Open Enrollment Period for Retirees (Fall 1984). These forms have been replaced by PEH 101-11-83 for use by active members.
