Employees who are on authorized leave of absence without pay for three months or more shall remit quarterly premiums in advance for each full quarter of such leave. For each period of authorized leave of absence without pay of less than three months duration the employee shall remit the full premium in advance. The employee shall mail the premium directly to PEEHIP and the employer shall certify that the employee was on authorized leave of absence.
