Contributions may be made by payroll deduction and such contribution shall be submitted to the PEIRAF and reported by the employer on a form prescribed by the PEIRAF administrative staff. Contributions may also be made by a lump sum payment submitted directly by the employee to PEIRAF on a form prescribed by the administrative staff. No lump sum contribution in an amount less than $100 will be accepted.
