The following are procedures for obtaining criminal history information from the Alabama Department of Public Safety in accordance with Act No. 92-676. The applicant's full name, race, sex, date of birth, and social security number. A release form signed by the applicant (you must use the Department of Public Safety release form, ABI46). A cashier's check, bank or postal money order in the amount of $25 (per applicant) made payable to the Alabama Bureau of Investigation.
