All applications for examinations and licensing shall be filed with the secretary-treasurer on forms provided by the Board. The application shall include all information required by statute and such other information as the Board may require by resolution, including, without limitation and in the Board's discretion, a history of arrests and convictions, a background check report, information about civil judgments and other information.
