Each applicant must request that a letter of verification be issued by the National Board for Certification in Occupational Therapy, stating that the individual has met the requirements for certification as an Occupational Therapist or as an Occupational Therapy Assistant and that the applicant has not been censured, put on probation, suspended or revoked by the National Board for Certification in Occupational Therapy. Said verification shall be sent directly to the Alabama State Board of Occupational Therapy.
