Transfer refers to movement from one service component to another within the same agency. It also refers to admission to another service or program within the same agency in addition to the program in which the client is currently being served. When transfer refers to movement from one component to another, the following standards apply:
(1) The client, the family, and other professionals, as appropriate, shall be included in the transfer decision and process. Efforts to include them in the transfer decision and process shall be documented.(2) Prior to movement from one program to another, the client and/or his family, as appropriate, shall be provided counseling concerning the advantages and disadvantages of movement and shall be afforded the opportunity to visit the setting to which transfer is being made.(3) If the client and/or family disagree with the program decision to transfer, they have the right to appeal.(4) A transfer summary shall be prepared and shall be entered into the client's record within 30 days following transfer. The transfer summary shall include: (a) A summary of significant events and progress during the period of services to the individual;(b) Specific recommendations for future services.(5) Steps (1) through (4) may be abbreviated in the event of an abrupt/emergency transfer.
